Youth Registration

For young people aged 13 to 17. A parent or guardian gives consent and confirms the registration with you.

Young people aged 13 to 17. If you are 18 or over, please use the young adults form.

A parent or guardian must give consent and enter their name at the end of the form. Please complete it together.

Safeguarding

All youth ministry activities follow the Diocese of Northampton safeguarding policy. Leaders are recruited and trained accordingly.

You can download the printed form and hand it in at the Parish Office instead.

Before you begin

This form registers a young person with the parish youth ministry. It takes about five minutes, and a parent or guardian should be with you to complete the consent section.

    Complete the participant details
    Name, date of birth, address, and a parent or guardian we can contact.
  2. 2
    Give the medical and emergency information
    So that leaders can look after your child properly during activities.
  3. 3
    Tick the consents and enter both names
    The parent or guardian and the young person each enter their full name, which acts as a signature.
Submitting this form sends the registration to the Parish Office. A member of the youth ministry team will then be in touch using the contact details you give.

Please use DD/MM/YYYY. The participant must be between 13 and 17 years old.

Data protection

Your personal details will be stored and used by the Parish for the purpose of running the parish youth ministry — organising activities, contacting you, and keeping participants safe. Medical and emergency contact details are held so that we can care for participants properly during activities, and are shared only with those leading them. Your details will not otherwise be disclosed outside the parish.
